All new patients to Montana Neurobehavioral Specialists
will receive a new patient packet, either by mail
or at their first appointment. This packet will
include our office financial policy and an insurance
coverage worksheet. We hope that this information
will help patients plan for their visit in our office.
Many insurance companies, including worker's compensation
carriers, require pre-authorization and/or referral
prior to obtaining specialty care. It is the patient's
responsibility to contact their insurer to determine
the need for a referral and/or pre-authorization.
Failure to obtain the referral and/or preauthorization
may result in lower payment or claim denial from
the insurance company.
Patient co-pays are expected at the time of their
appointment. If the co-payment amount is not known
the patient will be expected to pay 20% of the charges
for that visit. Our office will take steps to help
facilitate the processing of all claims, however
ultimately, any account balance is the patient's
responsibility. All accounts are expected to be
paid in full within 30 days of a statement being
issued.
For care of minors, the parent authorizing treatment
for a child will be the parent responsible for
the charges related to that care. If a divorce
decree requires the other parent to pay all or
part of the treatment costs, it is the authorizing
parent's responsibility to collect from the other
parent.

Patients needing to cancel appointments are requested
to contact our office with at least 24-hour notice.
This will allow us to offer the appointment to another
patient.
